The Clinical or Advanced Nurse Specialist will provide comprehensive transition management for high-risk patients discharged from the hospital to home. The Clinical or Advanced Nurse Specialist will provide assistance to patients across the continuum of care to improve patient safety, decrease hospital readmissions, and provide interventions for long-term management of chronic conditions. This role will assist in improving patient outcomes. The Clinical or Advanced Nurse Specialist acts as an educator and health coach for patients to attain self-efficiency.
A Master's Degree in Nursing or related area; and/or equivalent combination of experience/ training.
Registered Nurse license issued by the State of California.
Minimum of three (3) years of acute care experience to include knowledge of acute hospital discharge planning and patient education.
Proficient with information systems, computer skills and strong written and verbal skills.
Demonstrated ability to manage a high risk patient population towards a goal of self-management.
Proven ability and experience employing medication reconciliation and demonstrated expert nurse educator, critical thinking and nurse practice performance.
- Advanced Practice Registered Nurse, Certified Clinical Nurse Specialist, or licensure/ certification in another advanced nurse practice discipline.
- Clinical Nurse Leader certificate.
- Quality Improvement Methodology (Lean, Six Sigma, etc.)
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
