With a growing patient population and market share, the UC San Diego Health Care Management team is growing! The department is seeking an experienced Case Manager to join their leadership team.
At the direction of the department manager, the Care Management Supervisor shares responsibility for day to day operation of the care management department including direct supervision of the staff. Facilitates patient centered care, interactive management and collaboration within the care management department, interprofessional team and the community that serves our patient population.
The Supervisor has primary responsibility facilitating patient flow. They demonstrate expertise and the ability to implement timely transitions of care with plans that are safe, effective and focused on preventing unplanned returns to the ED and or hospital. This role models the mission, vision, and core values of the organization; supports evidenced based practice to insure optimal outcomes.

A Bachelor's of Science Degree in Nursing.
Registered Nurse license issued by the State of California.
BLS/CPR certification at time of hire with commitment to get BART within six (6) months of hire date.
A minimum of three (3+) or more years of RN Case Management experience.
Ability to effectively supervise a team and to manage the complex workflow and multiple priorities involved with care coordination and case management.
- Accredited Case Manager or Certified Case Manager (ACM or CCM).
- Recent work experience in acute case management.
- Recent experience leading a team, project, or committee.
- Inpatient acute care clinical RN experience.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
