This position will work a hybrid schedule which includes a combination of working both onsite at Greenwich Drive and remote.

Our Population Health Services Organization is growing. The Population Health team has a variety of programs that support patients. These programs include Utilization Management, UCSD at Home, Care Connections Hub, Quality and Disease Management Digital Health, along with student rotations to help health professionals of the future better understand population health at the patient care level. The teams coordinate with physicians, quality nurse specialists, nurse case managers (RN CM), social workers, and other healthcare providers to expedite medically appropriate cost effective care for patients, along with community resource linkages to address psycho-social and physical needs for patients at home for optimal patient centered care. The RN CMs apply clinical expertise and assessment when engaging patients in their medical, social and physical needs to create shared patients goals and advocate for medically necessary care and resources. Disease Management Digital Health focuses on patients who have a disease state that with the help of digital health tools and resources will help patients learn self-care for optimal health outcomes. A small set of patients will require high patient touch for care coordination, while most patients will be low touch as they use the various digital equipment, AI and texting resources. The designated CM remains the central point of contact for ongoing active and passive management based on patient needs.

The ideal candidate for the Case Management Manager over our Disease Management Digital Health program will possess a strong understanding of the key elements of Case Management and leadership skills to oversee a team of nurse case managers, medical assistants and digital integration health coaches who work with patients remotely helping them with digital health equipment integration, health education, program enrollment and self-care for optional disease management. Data transmits directly into the medical record, securely, allowing the CMs to identify which patients may need outreach or escalation back to their primary care provider. The ideal candidate will possess a strong solution focused approach, an understanding of lean process improvement, a strong knowledge of technical and digital tools, has a data driven and analytics approach to care and a passion to help patients of all ages learn how simple behavior changes will show positive results to help them in their goal for optimal health over their life span. Our program presently focuses on hypertension, diabetes, mild depression, and weight management with additional tools to support other disease states over the coming year.

This is an exciting opportunity for someone is interested in helping grow an innovative approach to remote patient monitoring and population health at a micro and macro level. Master's degree or higher, and previous work in acute or ambulatory care with experience are highly preferred. Background in case management / care coordination of patients with poorly controlled disease and / or population health preferred. This is a Monday-Friday position and will be a hybrid of on site and work from home option.

  • Nine (9) years of relevant experience, OR a Bachelor's Degree in Nursing (BSN) plus five (5) years of relevant experience; and/or equivalent combination of education and experience.

  • Registered Nurse (RN) license issued by the State of California.

  • Experience and proven success in ability to effectively supervise a team and to manage the complex workflow and multiple priorities involved with care coordination / case management.

  • Previous experience with nursing education and care coordination

  • Proficient skills and ability to navigate multiple computer systems in a fast paced environment.


  • Master's Degree in Nursing or Nursing Informatics.

  • CCM or ACM certification.

  • Previous work experience in acute care or ambulatory care setting.


  • Must be able to work various hours and locations based on business needs.

  • Employment is subject to a criminal background check and pre-employment physical.
