Under supervision of the Nurse Manager, this position collaborates with healthcare team to provide medical nutrition therapy to adult hemodialysis patients. Assesses nutrition needs and identifies nutrient requirements with regard to age- and disease-specific needs of patient. Develops and implements nutrition care plan for all assigned patients.
Fosters continuity of care among all phases across the continuum of care. Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Actively assesses, evaluates, and maintains professional development and competency related to assigned units.
Dietitians must possess the equivalent of a Bachelor's degree in Foods and Nutrition/Dietetics and have completed an Academy of Nutrition and Dietetics internship.
Must possess a Commission on Dietetics Registration (CDR) Registered Dietitian certification.
Minimum two (2) years Registered Dietitian experience.
Experience must include a minimum of one (1) year professional work experience in clinical nutrition as a registered dietitian.
Demonstrated experience developing and implementing nutrition care plans.
- Nephrology experience.
- Outpatient dialysis/chronic kidney disease and/or transplant experience.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
