DESCRIPTION

Under supervision of the Clinic Manager, the RDN (dietitian) collaborates with healthcare team to provide medical nutrition therapy to adult and geriatric patients. Registered Dietician in the ambulatory care setting holds independent clinics for monitored supervised diets to meet Bariatric Program requirements. Collaborates with multidisciplinary team including Bariatric Coordinator, Health Coach, Bariatric RN, NPs and peer Registered Dieticians to assist patients thru the Bariatric Surgical journey. Identifies patients at various nutrition risk levels based on criteria outlined by department. Assesses nutrition needs and identifies nutrient requirements with regard to age and disease-specific needs of patients.

Develops, implements and records in EHR the nutrition care plan for all assigned patients. Fosters continuity of care among all phases across the continuum of care. Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Manages the delivery of nutrition care in assigned units. Actively participates in medical rounds. Participates in department's special projects, performance improvement activities, and research.

Provides nutrition lectures and in-services to nutrition and medical staff as assigned. Actively assesses, evaluates, and maintains professional development and competency related to assigned units.

MINIMUM QUALIFICATIONS

  • Dietitians must possess the equivalent of a Bachelor's Degree in Foods and Nutrition/Dietetics and have completed an Academy of Nutrition and Dietetics internship.

  • Must be a Registered Dietitian (RD).

  • Two (2) years experience in comparable acuity level.

  • Applicants for positions in the Dietitian series are expected to possess the experience, skills, knowledge, and abilities essential to the successful performance of the duties assigned to the position.

PREFERRED QUALIFICATIONS

  • Demonstrated experience using EPIC.
  • Experience working in inpatient/outpatient settings.

  • Must be able to work various hours and locations based on business needs.
  • Employment is subject to a criminal background check and pre-employment physical.
