The Patient Record Abstractor is responsible for providing support for the Release of Information (ROI) requests for patient medical records. Patient Record Abstractor will log medical records request, determine records to be released by reviewing requestor information in accordance with HIPAA guidelines and obtaining pertinent patient data from various sources, including electronic, off-site, or physical records that match patient request. This role will respond and process requests from physician offices on a priority basis and fax information to the physician office in compliance with site facility policies and regulations. This role will respond to phone calls from medical and support personnel requesting medical records and escalate issues to manager that delay timely release of records.
The Patient Records Abstractor may be required to assign and track deficiencies, review all demographic changes made to the patient record, update and correct the electronic medical record and/or legacy platforms, and/or retrieve medical records for regulatory purposes. Additionally, this role will provide support to master patient index, scanning and indexing functions.
Graduation from High School or a GED with a demonstrated knowledge of the applicable medical terminology.
Two (2) years of related hospital clerical experience; one of which must be in the type of work to be performed; or an equivalent combination of education and experience.
- Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) certification.
- Professional fee coding certification, such as CPC or CCS-P.
- Healthcare Environment Experience.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
Job offer is contingent on successful engagement in the UC COVID-19 Vaccination program (fully vaccinated with documented proof or approved exception/deferral).
If applicable, life-support certifications (BLS, NRP, ACLS, etc.) must include hands-on practice and in-person skills assessment; online-only certification is not acceptable.
