This is a per diem position that is guaranteed 16 hours per week. Candidates will work their regularly scheduled days plus coverage up to 40 hours to include weekends on occasion.
Under general supervision of the Transplant Social Work Supervisor, candidate is responsible for ongoing pre-transplant psychosocial assessments and post-transplant follow-up of solid organ transplant patients to include liver, kidney, heart and lung. Candidates will also conduct living donor/recipient assessments. Candidate will work collaborative with a multidisciplinary team. Candidate will help facilitate transplant and caregiver support groups and participate in various transplant committees. Opportunities for advance credentialing and ongoing education in solid organ transplant.
Must Assist in coordination of discharge planning as needed and collaborate with transplant staff and social workers in other transplant programs. Position includes crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. Responsible for providing resources for local housing for out-of-town patients/families. The social worker is responsible for education and advocacy of patients, families and/or significant others. Candidate will participates in related department functions, such as community and program education.
A Master's Degree from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners.
At least two (2) years post MSW experience in a related field (CSWII) OR three (3) years post MSW experience and LCSW to be hired as a Clinical Social Worker III.
Previous experience working in both hospital and outpatient settings.
Knowledge and abilities essential for the successful performance of the duties assigned to the position.
Must have proven independent thinking, decision making, problem solving and prioritizing skills.
Excellent skills to communicate and influence effectively with all levels of staff, physicians, patients and external constituents, both verbally and in writing.
- Licensed Clinical Social Worker (LCSW) issued by the state of California.
- Previous experience working in a transplant center.
- Bilingual: English/Spanish.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
