Depending on experience of the selected candidate, this position will be hired at the CSW2 or CSW3 level. Less than two (2) years experience qualifies as CSW level; Three (3) years + LCSW experience qualifies as CSW3.
Under the general supervision of the Manager for Cardiovascular Operations and the Transplant Social Work Supervisor, candidates are responsible for ongoing psychosocial assessments of the patients provided care by the Adult Congenital Heart Disease team. This individual will facilitate patient and caregiver support groups and participate in various committees. Provide resources for local housing for out-of-town patients/families. Assists in coordination of discharge planning as needed. Collaborate with ACHD staff and social workers in other programs. Position includes crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Participates in related department functions, such as community and program education.
A Master's Degree from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners.
Must possess the knowledge and abilities essential to the successful performance of the duties assigned to the position.
Minimum of two to three (2 - 3) years post MSW experience.

- Licensed Clinical Social Worker (LCSW) - issued by the state of California.
- Bilingual in both English and Spanish.
- Prior experience working in a hospital setting.
- Prior experience working with pediatric patients and their families.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
